Senior Home Pathways is owned and run by clinicians: physical and occupational therapists who spend their days working with older adults, both in their own homes and inside senior living communities. We bring that hands-on expertise directly into helping your family.

So when we talk, we don’t just skim your answers. We review them with you, ask the clarifying questions that matter, and look closely at your parent’s medical history and their real functional abilities and challenges. Then, together with your family, we narrow it to one to three communities that genuinely fit, not the ones that pay the most or call the fastest.

Senior Home Pathways is owned and run by physical and occupational therapists. Our work has always been with older adults, in their own homes and in senior living communities, helping them stay safe, mobile, and as independent as possible. We’ve seen up close what makes one community a good fit and another a hard one.

We’ve also seen how overwhelming this moment can be for families, and how often they’re left to figure it out alone or handed off to a service that treats them like a number. So we built something gentler. When you reach out, you’re talking with a therapist who genuinely understands care and who takes the time to understand your family. That’s the whole idea: real guidance, from someone you can trust, when you need it most.
